Documentation Checklist

Please note, electronic files are only accepted in the following formats: Microsoft Word (DOC, DOCX) and Adobe Acrobat (PDF).

CV (Curriculum Vitae)/Resume

This documentation should detail your education and qualifications, work experience, extra-curricular activities, volunteer experience, interests, achievements, and skills.

Proof of Disability

Please visit the Frequently Asked Questions section for information on what types of disabilities are considered eligible. Proof of disability should be in the form of a letter from your accessibility centre or a letter from a medical professional indicating that you have a disability. However, the type of disability should not be mentioned in the documentation to respect your privacy and confidentiality. Any other type of documentation will not be accepted.

Academic History

Transcripts (including unofficial transcripts) printed from student accounts will be accepted as long as they include the student's name and/or student number. These transcripts must not show signs of tampering or modification in any form.

References

You must include (1) academic reference and (1) community reference. The academic reference is to be written by an individual who is familiar with the applicant’s academic history.  This individual should be a professor, instructor, or someone who is familiar with the applicant's academic performance.  They must not be a family member.

The community reference is to be written by an individual who is familiar with the applicant’s extracurricular activities and outstanding characteristics. This individual should be a former employer or representative of a community organization.They must not be a family member.

Applicants must indicate the exact name and email address of their referees in their application.Referees will be sent a reference form to fill out to their email address.

**Applicants should advise their references to check their spam folder for the link to the reference form.
