Request for Feedback on the CSLP Application Form

The Canada Student Loans Program (CSLP) is looking for feedback on two of its application forms for the Repayment Assistance Plan for Borrowers with a Permanent Disability (RAP-PD).

• The Verification of Permanent Disability form, and
• The Permanent Disability Related Expenses form

The purpose of this request for feedback is to ensure that these forms are using appropriate language and are fully accessible for all users.

If you would like to participate in the review and provide your feedback please contact the CSLP at info@canlearn.ca by February 15, 2010 The CSLP will then provide you with the forms and will ask that you return your feedback within one week.
